Examples of Care Needed
Major Disaster — Home Fire — lost clothes, personal belongings.
Major Medical Needs — When you are out of work for an extended period of time and need help to sustain and pay for housing, utilities, etc. This would be a car accident with major injuries, a heart attack or other illness that keeps your from work, cancer treatments, this would help towards rent, utilities, and other bills for living expenses or medical treatments.
Family Funeral cost where the family did not have enough to pay for the funeral.
Medical needs for immediate family — Husband, Mom, Dad, Kids go through something mentioned above and you might need to be out of work for a long period of time, unable to sustain due to medical cost from cancer, major illness. This would help towards rent, utilities and other bills for living expenses or medical treatments.

Characteristics of Connect in 40
It is a one-time donation, except as special circumstances may dictate
Required supporting documentation includes filling out the below online donation request including
— Letter summarizing the situation and the financial hardship created by it
— The amount of the donation requested
— Proof of hardship – Bank statements, copies of bills for medical (or other) and/or living expenses which has been incurred as a result of the situation
— Signed physician’s statement (if this is a medical situation)
— If the applicant has health insurance, please provide a summary of all claims from the insurance provider indicating the patient’s responsibility for the charges
